What it is
Three behavior forms for kindergarten through grade 8: a weekly chart where students circle a 3, 2 or 1 for each subject and day, an incident documentation log, and a tally sheet for counting one behavior across the week.

Ways to use it
- Give a student the weekly chart on Monday and have them circle a 3, 2 or 1 after each subject, then use the key to talk through the hard days.
- Keep the documentation log on a clipboard and give each incident its own line, noting the date, time, action taken and follow up, so you have notes ready for a family or admin conversation.
- Pick one behavior, such as calling out, and add a tally mark every time you see it, to find which times of day need a change.
